More Information & Referral Forms

Please see "Our Services" for information pertaining to your referral pathway. If you are seeking to refer a Client:

 1. Under the NDIS (excl. agency-managed plans), please download the referral form below and email this to intake@abilitypsychologyservices.com. To expedite the processing of your referral, please book your Intake Call via: https://clientportal.powerdiary.com/clientportal/intakecall. Appointments can be offered in clinic (North Lakes) or via outreach, upon request (e.g., home-visit or school-visit). 

2. For assessment only, please download the referral form below and email this to assessmentteam@abilitypsychologyservices.com. Assessment are offered in clinic (North Lakes) and via outreach, where suitable (e.g., home-visit or school-visit). 

3. And you are a GP seeking to refer a Client, please send your referral via Medical Objects and/or via email to intake@abilitypsychologyservices.com. Please note that we do not offer bulk-bill services. Our clinic is located in North Lakes, QLD (near Westfield). 

4. Privately or via Private Health Insurance please make an appointment via our Client Portal or submit an enquiry to make an appointment. Please see the 'Contact Us' tab for further detail. Please defer to your insurer's PDS regarding eligibility for access and coverage. 

5. Via an approved WorkCover QLD claim, please email intake@abilitypsychologyservices.com or submit a 'Contact Us' form on our website with the details of your enquiry. 

6. Under an existing Third Party Service Agreement (e.g., an established on-site service model), please download the referral form and email this to your respective point of contact.

Alternatively, please contact our office during standard business hours (AEST) on 1300 229 906 and a member of our team will be happy to help you.
