1. Under the NDIS (excl. agency-managed plans), please download the referral form below and email this to intake@abilitypsychologyservices.com. To expedite the processing of your referral, please book your Intake Call via: https://clientportal.powerdiary.com/clientportal/intakecall.
2. For assessment only, please download the referral form below and email this to assessmentteam@abilitypsychologyservices.com.
3. Under an existing Third Party Service Agreement (e.g., an established on-site service model), please download the referral form and email this to your respective point of contact.
4. And you are a GP seeking to refer a Client, please download the GP referral form below and email this with any attachments (e.g., MHCP) through to intake@abilitypsychologyservices.com. Please note that we do not offer bulk-bill services.
5. Privately or via Private Health Insurance, please download the 'General' Referral Form and email this to intake@abilitypsychologyservices.com or alternatively, submit a form via our 'Contact Us' form of the website with the details of your enquiry. Please defer to your insurer's PDS for queries surrounding eligibility for access and coverage.
6. Via an approved WorkCover QLD claim, please email intake@abilitypsychologyservices.com or submit a 'Contact Us' form on our website with the details of your enquiry.
If you have any questions or are enquiring about other services not referenced in the above referral process, please head over to our 'Contact Us' form and lodge your enquiry. Alternatively, please contact our office during standard business hours (AEST) on 1300 229 906 and a member of our team will be happy to help you.
